Any parent that has not received their Fitness Credit Tax Receipt can complete the form below. We will e-mail a receipt within five working days. Please note that if you paid online you receive a receipt through the online system. Only athletes that registered by mail and paid by cheque should be requesting this receipt.

IMPORTANT: We only forward receipts AFTER a program is over. Therefore, club volleyball receipts are forwarded in June, Fall Camp receipts are forwarded in January and so on.
